Our forms are now available on ePACT Network. Please watch for an email invite from Monmouth County Park System via ePACT Network to create your account.

If you do not receive an email from the ePACT network, please check your SPAM folder. If it had not gone to your SPAM folder, please contact us to ensure that we have the proper email address. If you attended camp with us last year and you have an account, please watch for an email to review and update your account information.

Camp information, as well as special accommodations forms (if needed) must be completed and returned prior to attending camp. Special accommodations forms are found below. If you have questions regarding camp forms, call 732-842-4000, ext. 4312.

If you signed up children in addition to your own for camp, please make sure the parents/guardians of the other children contact us so we can send them an invite to ePACT to create their account to fill out their camper’s information.

When completing the information on the ePACT account, it is only necessary to fill out the information marked with an asterisk*. Please complete all of the required information under each tab. Please upload any special accommodations forms you may have.

Camp forms will be completed on the ePACT Network and submitted to us.  This includes the following information:   

  1.  Participant/Pick-up Authorization Information

  2.  Assumption of Risk/Release Form

  3.  Participant Health & Accommodations Information

  4. Behavior Management Policy

In the event your child requires special accommodations, has a food allergy or requires medications be taken during camp, these additional forms (please note that these forms do not apply to Therapeutic Recreation Camps) must be completed and uploaded to your ePACT account in the Shared File section:  

*The Medication Authorization form must be completed and submitted along with the Allergy Action Plan and School Action Plan Forms.

Please note that these forms are PDFs and require Adobe Reader to open.

If these forms are not completed your child may not be allowed to participate in camp. You only need to complete one set of these forms regardless of the number of camps your child is registered for.
